Transportation Permit Applications
The applications accessible from this page are a convenient way to view and to fill-out applications on-line. The actual submission of applications on-line is not yet available through this web site, but is planned for a future version. You can, however, access the desired applications here, fill them out on-line through this web site, print the completed applications, and then mail them to the RMA Transportation Branch. The address of the RMA Transportation Branch is shown below. (If you prefer, you can just print the blank applications from this site, fill them out by hand, and mail them.)

APPLICATION FOR TRANSPORTATION PERMIT...REQUIREMENTS
...must be filled out in full, including the signature at the bottom of the form.
CERTIFICATE OF INSURANCE is required. A minimum liability of $500,000 is required and the following phrase must be typed verbatim on all certificates of insurance:
"THE COUNTY OF TULARE, ALL OF ITS OFFICERS AND EMPLOYEES ARE NAMED AS ADDITIONAL INSURED."
ANY REQUIREMENTS OR REGULATIONS YOU FEEL UNJUST MAY BE APPEALED TO THE COUNTY BOARD OF SUPERVISORS.
