Loan Application

Use this form if you are applying to take a loan against your account balance.  All repayment of loans is done by payroll deduction.

Loan Offset Form

If you leave employment with the County with an existing loan balance still owed, complete this form to request that the remaining unpaid balance be treated as taxable income to you.

Loan Payment Change Form

Use this form if you have an outstanding loan and are going out on an unpaid leave or returning from an unpaid leave so that loan payments can temporarily be stopped, or so that it can be restarted after the leave, and so the loan will not automatically go into default as a taxable event.

Loan Prepayment Request Form 

Use this form if you have an outstanding loan that you want to prepay in full.  Call Empower Customer Service and they can give you the loan payoff amount.
