This plan is only available to members enrolled in an Anthem Blue Cross plan.
Vision insurance coverage is provided to eligible employees and qualified dependents who are enrolled in an Anthem Blue Cross medical plan. The vision plan is provided through Vision Services Plan (VSP).
The program provides for an eye examination and lenses every 12 months, and frames once every 24 months. Participants are responsible for a $10 co-payment to the participating doctor for the eye examination and an additional co-payment of at least $25 for lenses and/or frames. Benefits are reduced if a non-VSP member doctor is used.
