Mining and Aggregate Production

The following forms are required for reporting production and operational details of mining and aggregate operations. This form is required for property owners and operators extracting aggregate materials such as sand, gravel, stone, limestone, and clay. It collects information on production, sales, operating costs, reserves, and other relevant details for the previous calendar year. Operators must submit a separate report for each location.

Mining Production Report (BOE-560-B)

This form applies to operations extracting minerals such as diatomite, iron, rare earths, gold, talc, tungsten, and other mined materials. It captures production and sales volume, gross income, royalties, reserves, operating costs, and lease details. Operators must submit a separate report for each mining location.

Mining Claim Production Report (BOE-560-C)

This form is required for holders of mining claims. It records information on claim activity, mineral extraction, and associated income. Claimholders must also report any acquisitions, sales, or abandonments that occurred during the previous calendar year.
