BOE-267-H – Welfare Exemption Supplemental Affidavit, Housing – Elderly or Handicapped Families

This form is for nonprofit organizations providing housing for low- and moderate-income elderly or handicapped families. It is required to document household income eligibility to qualify for a Welfare Exemption.

BOE-267-L – Welfare Exemption Supplemental Affidavit, Housing – Lower Income Households

Nonprofit organizations or eligible limited liability companies that provide housing to lower-income households must file this affidavit to qualify for a Welfare Exemption. 

BOE-267-L1 – Welfare Exemption Supplemental Affidavit, Low-Income Housing Property of Limited Partnership

This affidavit is for limited partnerships with a nonprofit managing general partner seeking a Welfare Exemption on properties providing low-income housing. 

BOE-267-L2 – Welfare Exemption Supplemental Affidavit, Housing – Lower Income Households – Tenant Data

Organizations must use this form to provide tenant income data when applying for a Welfare Exemption for lower-income housing properties. It is used to confirm that residents meet the income qualifications.

BOE-267-L3 – Welfare Exemption Supplemental Affidavit, Households Exceeding Low-Income Limits – "Over-Income" Tenant Data (140% AMI)

This form is required for properties where household incomes exceed the lower-income threshold but remain under 140% of the area median income (AMI). It allows continued Welfare Exemption eligibility for rent-restricted units.

BOE-267-O – Welfare Exemption Supplemental Affidavit, Organizations and Persons Using Claimant’s Real Property

Property owners must use this form to disclose information about other organizations or individuals using the property.

BOE-267-R – Welfare Exemption Supplemental Affidavit, Rehabilitation – Living Quarters

This affidavit is for organizations providing housing for persons undergoing rehabilitation. It collects data on housing occupancy, lengths of stay, and rehabilitation programs to support the Welfare Exemption claim.
