This form is used to notify the Assessor’s Office of any changes affecting a property’s eligibility for the Welfare Exemption under sections 214.15 or 231 of the California Revenue and Taxation Code. Organizations granted this exemption must confirm their continued eligibility each year by returning a completed form. If the property is no longer owned or is used for non-qualifying activities, the exemption may be modified or terminated to avoid potential escape assessments and penalties. The form must be completed and returned by June 30 if there has been a change in ownership or use.
